Harvard Pilgrim CT Policy Updates for 3D Mammography Coverage

November 2017 ~

Effective for dates of service beginning January 1, 2018, Harvard Pilgrim will cover 3D mammography (digital breast tomosynthesis (DBT)) for screening or diagnostic purposes for members of its Connecticut plans. This change is in compliance with the state legislation which, as of January 1, 2017, requires all health plans in the state to cover DBT.

In addition to 2D screening and diagnostic services, providers in Connecticut may now bill for:

• Add-on CPT code 77063 – Screening digital breast tomosynthesis, bilateral (list separately in addition to code for primary procedure).
