BCBS Massachusetts Medical and Pharmacy Policy Updates

October 2018 ~

Blue Cross Blue Shield of Massachusetts (BCBSMA) has published an updated list of its new, revised, and clarified medical policies and pharmacy medical policies to become effective November 2018.

These policy updates are as follows.

New 2018 Category III CPT Codes

All category III CPT Codes, including new 2018 codes, are non-covered unless they are explicitly described as “medically necessary” in a BCBSMA medical policy. To search for a particular code, go to BCBSMA’s Medical Policy page and type the code in the search box on the page. Consult the coverage statement of any associated medical policy. If there is no associated policy, the code is non-covered.
