BCBS Vermont New, Revised and Deleted Codes Released

September 2018 ~

Following Blue Cross and Blue Shield of Vermont’s (BCBSVT) review of the CPT® and HCPCS additions, deletions and revisions for October 1, 2018, the insurer has made several changes involving prior approval, investigational services, and unit designation. These changes are as follows:

Prior Approval

The following new October 1, 2018 codes require prior approval:

  • New CPT® codes: 0063U, 0070U, 0071U, 0072U, 0073U, 0074U, 0075U, 0076U
  • New HCPCS code: Q5108*, Q5110*

*prior approval is through Express Scripts, Inc.

Investigational

The following new October 1, 2018 CPT® codes are considered investigational:

  • New CPT® codes: 0062U, 0067U, 0069U, 0078U
  • New HCPCS Code: C9750

Unit Designation for New Codes

The following new October 1, 2018 CPT®/HCPCS have the following unit designations:

  • Q5108 – Multiple
  • C9033, C9034 – Multiple
  • C9750 – Single
  • Q5110 – Multiple
  • G9978-G9987 – Single
  • 0062U-0065U – Single
  • 0066U – Multiple
  • 0067U-0079U – Single

Unit Designation Changes

The following CPT®/HCPCS/CDT have changed from a single unit designation to a multiple unit designation as of October 1:

  • 11440, 59025, 72100, 73100, 73560, 73562, 73564, 82397, 85347, 85384, 85613, 85732, 86317, 86611, 86635, 86753, 86900, 86901, 87176, 87177, 87205, 87209, 87529, 87899, 88172, 88173, 88311, 89051, 89060, 95886, A4454, C1726, C9399, D0230, D2930, G0410, P9016

The CDT code D9222 has changed from a multiple unit designation to a single unit designation as of October 1.

Services billed for dates of service on or after October 1 require the correct billing of unit designation.
