CMS Releases New ABN (Advanced Beneficiary Notice of Non-Coverage) Form

September 7, 2011 – CMS has issued a new ABN form (CMS-R-131) that is to be used by physicians, practitioners, independent laboratories and suppliers in situations where Medicare payment is expected to be denied.
 
The new form will contain the date 3/2011 in the lower left hand corner. Providers should begin to use these forms immediately.  The deadline for the discontinuance of the old form is November 1, 2011.  Any forms submitted as of November 1, 2011 with the date 3/2008 in the lower left hand corner will be considered invalid.
